Maintains awareness of client’s physical, mental and spiritual well-being
Transfers clients per care plan.
Performs the following client services: bathing, assisting in skin care (reporting any skin changes), assisting in foot care, oral hygiene, shampooing, filing and cleaning nails, dressing, shaving, and monitoring variations of the client’s physical baseline.
Ensures adequate nutrition and hydration of client. performs basic ostomy and incontinence care.
Performs limited home management tasks in the immediate client environment, which may include: meal preparation, washing dishes, light laundry, light housekeeping, and bed making with linen changing.
Works collaboratively with other community health care providers.
May provide lengthy service visits, extended hours or overnights.
Participates as a member of the Hospice Interdisciplinary Team to address problems.
Responsible for providing care in a skilled nurse facility.
Perform selected activities as written on the care plan under the supervision of the nurse or therapist including:
Ostomy care.
Responsibilities of the Hospice Aide:
Adheres to Agency infection control policies.
Adheres to the client’s written care plan and informs the supervisor or case manager of any change in condition or family situation.
Seeks assistance and notifies the agency when a change of schedule is required or when there appears to be a safety risk for self or others.
Assists in personal care and home management relating to comfort, care, and safety of a client in the home.
HOSPICE AIDE’S MAY NOT:
Make judgment or give advice on medical or nursing problems.
Handle finances of clients or their representatives, including any banking functions, handling cash and valuables, or purchasing tobacco, alcohol or lottery tickets.
Perform housecleaning, except light housekeeping directly relating to client care.
Care for tracheostomy.
Provide clients with their personal telephone numbers.
Work directly for clients on a private duty basis.
Perform any activity that will jeopardize a client, the agency or a staff member.
Ability to read, write and speak English.
Home Health or Hospice Experience preferred
Registration/Certification
Home Health Aide Certificate required
Applicants pre 8/14/90 - Completion of 60 hour Home Health Aide Training in DPH approved program. No additional training is required.
Applicants post 8/14/90 - Completion of 75 hour Home Health Aide Training following HCFA guidelines. No additional training is required.
Applicant credentialed as home health aide through out-of-state program meeting HCFA regulations: No additional training is required. Agency must verify prior employment with Medicare-Medicaid certified home health agency. Handling light materials. Requires sustained visual attention and concentration. Potential exposure to blood, body fluids, and other infectious materials. Physical effort required to lift, carry, push and pull light to heavy equipment, objects or supplies.
