In this contractor role, the MD will be responsible for establishing the Tapestry Health patient relationship for our chronic care management services. Document patient encounters accurately and thoroughly within our platform at time of visit.
Active, unrestricted, Medical License in Texas
Must have reliable transportation to travel to assigned buildings.
Commitment to providing patient centered care and improving health outcomes.
Familiarity with electronic health record (EHR) systems and healthcare technology.
Ability to travel within 1-2 hours of assigned region
COMPENSATION STRUCTURE:
$25 per patient visit, paid upon completion of assigned facility census.
Mileage reimbursement available.
