* Careline Nurse Practitioners provide primary and/or consultative professional care in a variety of care settings. The Nurse Practitioner practice will focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life.
_Conducting medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment
* Communicating significant findings, problems, and changes in condition or environment to the clinical coordinator, the physician, family, facility and other personnel involved with patient care
* Assessing health status by obtaining relevant health and medical history.
* Performing a physical exam based upon the age and patient history as per the Standard Care Arrangement with a collaborating physician
* Ordering, interpreting and evaluating laboratory tests and other diagnostic studies to assess patient’s clinical problems and health care needs
* Performing diagnostic of therapeutic interventions consistent with certification, law and state medical privileges
* Prescribing and monitoring medications as allowed by prescriptive authority through the appropriate State Board Agency
* Referring patients to physicians as appropriate for consultation or for specialized health resources and treatment
* Ensuring continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas
* Providing patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits
* Holding case conferences with families when appropriate to advance the care plan
* Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
* Nursing skills as defined by accepted nursing standards
* Must be able to lift at least 35lbs and bear the weight of an average adult effectively
* Demonstrates ability to assess and respond to the needs of patients, families and caregivers in skilled-nursing facility settings
* Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
* Office Location: 4760 Fashion Square Blvd, Saginaw MI
* 5,000 Sign On Bonus
* Office hours: 8:00 AM – 5:00 PM, Monday through Friday
