Maxim Healthcare is looking for a Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) who assumes responsibility and accountability for the application of the nursing process and the delivery of patient/client care. The LVN/LPN is also responsible for providing services, as directed by contract and/or program, while under the supervision of a contract/program Registered Nurse or clinical designee. The LVN/LPN provides service in accordance with Maxim’s philosophy, standards and policies.

Essential Duties and Responsibilities:

Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
On each assignment, provides skilled nursing care/services in accordance with prescribed orders
Identifies and documents changes in patient/client physiological and psycho-social condition
Notifies Director/Supervisor, and contracted customer designee if applicable, when:
Physician notification is required in order to obtain necessary orders for intervention(s), per state regulations
Changes in patient/client condition have occurred
Adheres to Maxim policy, procedure and applicable protocols
Identifies and prioritizes actions based upon patient/client care requirements and/or contracted customer requirements
Ensures that documentation reflects patient/client collaboration including participation in plan of care creation and maintenance, physician correspondence/collaboration, expected patient/client outcomes, nursing interventions employed, and other items as required by contracted customer
Performs/provides interventions, treatments, and medication administration, according to identified priorities, the plan of care, facility policies and patient/client care outcome standards as applicable
Monitors and documents effectiveness of interventions, treatments, and medications
Assures patient/client safety at all times
Monitors the safety and effectiveness of the care/services environment and equipment used, including universal precautions
Executes consistent, effective interpersonal communications and maintains positive, professional relationships with patient/client and contracted customer stakeholders
Adheres to the following professional practice standards:
Nurse Practice Act
Standards of Nursing Practice
Standards of Patient Care
Standards of Accrediting Body
State and Local Laws
Patient Bill of Rights
Policies and procedures of contracted customers
Adheres to Maxim policy, procedure and applicable protocols
Attends case conferences, team meetings and education in-services, as required
Performs other duties as assigned/necessary

Minimum Requirements:

Graduate of an accredited practical nurse or vocational nursing program preferred
Currently licensed as a Licensed Practical Nurse or Licensed Vocational Nurse license in specific state required
Complies with accepted professional standards and practice required
One year prior professional nursing experience preferred
Willingness to participate in skills advancement training or novice nurse program if applicable
Current CPR, as applicable, for direct patient care
PPD or chest x-ray if applicable
Successful completion of Maxim Orientation and Compliance Training, and all other assigned trainings as applicable required
Successful completion of Maxim’s Competency Program required
Proficiency in the English language required
Current Health Certificate (per facility contract or state regulation) required
Must meet all federal, state, local, and contract requirements
