**Hybrid Role: 3 days of field work and 2 days of remote documentation work - must be local to McAllen/Mission, TX area **

Job Responsibilities / Description:

  • Daily responsibilities:
    • Assist with member tasks – follow up with members regarding coordination of care
    • Follow up with providers and doctors’ offices.
  • Performance Expectations/Metrics:
    • Provided a monthly caseload (20 per month)
    • Required to follow up with members on a weekly basis
  • What previous job titles or background work will in this role?
    • Home health experience
    • Hospital experience.


HOURS : Monday to Friday - 8:00AM - 5:00PM
PAY: $38-$48 hourly (determined on experience)

Required Education & Experience

  • At least 1 year of Case Management experience
  • Basic computer skills (Microsoft Word, Excel)
  • RN license
  • Pediatrics and/or home healthcare.

Preferred Experience:

  • Highly Preferred (but not required): Bilingual in Spanish
  • BSN Degree highly preferred
