Overview

At St. Luke’s, our dedicated team of Home Health Nurse Liaisons strive to build a positive, supportive, and inclusive culture that delivers exceptional patient experiences.

As a Nurse Liaison, you will foster the relationship between patients and the facilities to identify the next level of care. You will perform clinical assessments and screening, establish patients' eligibility for care, communicate with families, and provide education on the benefits of at home care. You will coordinate care within the Twin Falls area.

What you can expect from this role

Full Time Days (Mon-Fri 8 hour shifts) with rotating weekends Works autonomously and with multidisciplinary team Collaborate to design individualized care for adult patients

Qualifications

Education: Nursing Degree Experience: 2 years of RN experience; Home Health or Acute Care experience helpful Licenses/Certifications: Current RN License in the State of Practice. Current Basic Life Support (BLS) Certification Provider Certified through American Heart Association or American Red Cross Valid Driver's License and proof of Auto Insurance
