Help us help others:

We are searching for a Mental Health Care Coordinator who will assess needs, develop treatment plans, and effectively collaborate with individuals and their family members. The Care Coordinator will assess strengths and needs and, in collaboration with the individual, develop a person-centered recovery plan. The Care Coordinator will provide skills training, crisis prevention/management, linkage, and referral to support goal progress for children and families living with mental health challenges. This compassionate, caring, and supportive team member will contribute to the skills improvement, achievements, and overall lifetime potential success of a child and their state of wellbeing.

You will get to:

Assess individual needs and collaborate with them to develop and execute effective recovery and life management plans. Provide relevant skills training to improve daily life. Collaborate with service providers and agencies to be a liaison for effective care and positive outcomes. Write and document articulately while using correct grammar so that documentation is clear and valuable. Maintain compliance with all legal requirements and company policies and procedures. Consistently meet with individuals to maintain progress as well as compliance.
