Bridges Healthcare, Inc. is seeking a Peer Case Manager for our Behavioral Health Home/Community Support Services programs.

$21.07 to $27.08 per hour commensurate with experience

PURPOSE and POSITION OVERVIEW

Support activities and programming that focus on individual's recovery by increasing their overall quality of life and independence. Provide intensive skill-building and motivational support that will help identified individuals pursue goals related to health and wellness, employment, education, community integration and use of natural supports. Serve as an advocate, through personal experience, to assist with individuals in their recovery plan


DUTIES and RESPONSIBILITIES

  • Facilitate recovery-focused activities to motivate consumers toward improving their quality of life.
  • Assist consumers in the development of a crisis plan and advanced directives for emergency/crisis situations.
  • Provide advocacy and education on treatment barriers and stigma.
  • Network and collaborate with community resources and providers.
  • Assist individuals in setting goals related to health and wellness activities.
  • Completes required paper and electronic documentation in a timely manner.
  • Assist in scheduling and coordinating medical appointments.
  • Collaborate with Nurse Care Manager and other identified providers to help individuals manage chronic illnesses.
  • Supervise, coach and provide guidance to supervisees.
  • Perform any other department or agency related duties as directed by supervisor.

QUALIFICATIONS and COMPETENCIES

Bachelor's degree preferred; Certified Peer Support Specialist program completion required

Ability to work flexible schedule (Monday through Friday); potential availability needed for evenings, weekends and holidays.

Knowledge of behavioral health issues with an understanding of community integration strategies and techniques.

Good communication and documentation skills.

Computer literacy.

Ability to prioritize and organize schedules/demands.


Bridges is STAR-LRP AND HRSA Approved, and some positions may be eligible for loan repayment through these programs.
