Position Highlights:
Region: Orange CountySchedule: Monday – FridayIntroduce the member’s medical group, Optum California to members, family members and caregiversCounsel and educate patient and families regarding health plan benefits, health plan network and other programs and tools available to help improve quality of lifestyleConduct assessments and annual wellness exams with patient (clinic, IPA office, SNF, home setting or virtual). Document chief complaint, previous medical, family, and social history, review of systems, examination, assessment, and appropriate plan of care. Responsible for the coordination of care with specialists and appropriate ancillary servicesComplete all documentation and paperwork in a timely manner. Required Qualifications:
Master’s degree from four-year college and/or a professional certification beyond a four-year collegeCertificate of completion from accredited Nurse Practitioner ProgramCurrent AANP Certification required Unrestricted CA NP licenseCurrent CA DEA certificate required prior to start dateActive BLS certificationEMR ProficientExcellent patient care and time management skillsFlexible, multi-prioritizing skills and time management skillsAbility to work independently and without direct supervisionPreferred Qualifications:
Fluency in Spanish 1+ years experience; preferably in outpatient/managed care settingWorking knowledge of managed care, health plans, medical groups, IPA networksCalifornia Residents Only: The salary range for this roleis $, to $, per year. Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement.
