GENERAL STATEMENT OF JOB

The MFP Transition Manager oversees the MFP Transition Coordination process for individuals who are residing in a Skilled Nursing Facility (SNF) and meet criteria to be a part of the MFP program. Transition Coordinators lead participants and the planning team through a planning process and ensures that participants have supports in place to live successfully in the community. The MFP Transition Manager ensures the effective implementation of the transition services. This role requires a deep understanding of person-centered planning, Medicaid, and community-based services. The Manager will supervise the transition process, ensuring coordination and collaboration among various stakeholders and uphold the quality of transition services. Strong collaboration with other community partners is essential for quality transition planning.

This Manager must be able to independently identify, implement and assess system and organizational priorities relative to Olmstead Initiatives. The Manager will demonstrate timely responsiveness to team members, internal LME/MCO staff, members, families, and external partners; communicate clearly and concisely through both verbal and writing skills and case formulation. Additionally, this Manager must demonstrate strong interpersonal skills, and the ability to make prompt independent decisions based upon relevant facts and established processes. Problem solving, negotiation and conflict resolution skills are essential to balance the needs of both internal and external customers.

The manager will serve as primary point of contact for general communications between the Department and Transition Coordinators. In addition to supervising Transition Coordinators, the manager will ensure adequate oversight to ensure statewide coverage. The manager will review transition tools and reports for accuracy, completeness, and quality. The manager will ensure the delivery of quality transition practices, manage transition startup funds. The manager will serve or designate a representative as MFP’s housing liaison on MFP housing meetings,

attend meetings as directed by the Department, serve as the referral source for targeted/key housing applications, serve as the referral source for targeted/key housing applications, and will attend November MFP Roundtable Meeting each calendar year.

ESSENTIAL JOB FUNCTIONS

Supervision and Staff Management:

Supervise MFP Transition Coordinators, ensuring they fulfill their duties effectively and within the required timelines. Conduct monthly supervision and implement disciplinary actions as necessary. Ensure staff have necessary training and resources to perform their job duties. Review applications for employment, conduct interviews, and make hiring recommendations as needed. Evaluate staff performance and provide feedback and guidance for improvement.

Program Implementation and Oversight:

Develop, implement, and oversee transition programs that assess the strengths and needs of members. Ensure programs align with the Vaya MFP response and all conditions of the MFP contract. Ensure all required documentation and reports are completed accurately and timely.

Training and Education:

Will conduct MFP outreach and education plan, to include a broad spectrum of stakeholders across North Carolina. Will conduct new employee MFP training.

Collaboration and Communication:

Facilitate effective communication and collaboration across the agency related to MFP In-Reach and transition activities. Participate in LME/MCO meetings, community partner advisory boards, and workgroups as needed. Maintain ongoing communication with internal staff, external partners, members, and their families to support successful transition planning. Promote goodwill on behalf of the LME/MCO and address any issues or concerns promptly.

Documentation and Reporting:

Ensure accurate and clear documentation of in-reach and transition progress for each member. Submit formal recommendations and reports on a timely basis to inform local, state, and federal agencies. Maintain comprehensive records of all contacts and interventions with members.

Other Duties as Assigned:

Perform other related duties as required.

KNOWLEDGE, SKILL & ABILITIES

This position plays a vital role in advancing the mission of the MFP program, supporting individuals to transition from institutional settings to living independently in their communities with the necessary supports. The ideal candidate will have a strong background in human services, excellent organizational skills, and a commitment to promoting the well-being of vulnerable populations.

QUALIFICATIONS & EDUCATION REQUIREMENTS

A Bachelor’s degree from an appropriately accredited university or college in public service administration, psychology, sociology, or social work or a human service programmatic field, preferably with coursework in human service planning and four years of experience in directly related human services program; 

OR

A Bachelor’s degree from an appropriately accredited university or college in public service administration, psychology, sociology, or social work or a human service programmatic field, preferably with coursework in human service planning and four years of administrative or consultative experience in a human service program; 

OR

A Master’s degree from an appropriately accredited university or college in a related human services field and two years of supervised clinical experience; or equivalent combination of education and/or experience.

Direct related experience is defined a human services experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy, and treatment planning.

The incumbent should have experience working with children, adolescent and adults with MHSU and IDD diagnoses.

Licensure/Certification Requirements:

Travel is required and incumbent must have a valid NC driver’s license.

PHYSICAL REQUIREMENTS

Must be able to perform the basic life operational functions of standing, walking, fingering, talking, hearing, and repetitive motions. Must be able to perform sedentary work exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or otherwise move objects. Visual acuity to prepare figures and data, operate a computer, proof work, and do extensive reading.


RESIDENCY REQUIREMENT: This position is required to reside in North Carolina or within 40 miles of the North Carolina border.

SALARY:  Depending on qualifications & experience of candidate. This position is exempt and is not eligible for overtime compensation.
