As part of an adult moderate CCBHC team, Nurse Care Manager will provide the following:

Serve adult clients, age 18 and older, that are diagnosed with a severe mental illness, substance use disorder, or co-occurring disorder. Combination of office and/or field-based services with potential transportation of clients using agency or personal vehicles as determined by needs of consumers and/or program. Meet clients where they are in terms of current mental health symptoms and treatment preferences Provide med reconciliation Coordinate care and communicate freely with consumers’ primary care providers Provide nursing assessment and wellness services for consumers as appropriate. Provide follow up and coordination from hospital or ER visits. Work collaboratively with community partners Provide services in-person or via telehealth platforms Gather and complete all CCBHC data metrics as determined To also include all duties and responsibilities outlined in primary RN/Nurse Care Manager job description.

QUALIFICATIONS:

Requires completion of Registered Nursing (RN) Program and current licenses as an RN in the State of Oklahoma. 

This job is classified as a “safety-sensitive” position as defined by the Oklahoma Medical Marijuana and Patient Protection Act. Possession of a medical marijuana license will not exclude any applicants; however all employees are subject to the Family & Children’s Services Drug Free Workplace Policy.

Applicants will have previous background working in community mental health, inpatient psychiatric facilities, home health, or similar settings. Strong candidates with a passion for helping individuals with severe mental illness will also be considered.
