This is more than just a job; it’s an opportunity to make a real difference in people’s lives Position Details Hours: Full-time | Days, Monday - Friday | Weekend and holiday rotation required. What You’ll Experience At Interim HealthCare, you’ll enjoy: Flexible scheduling for patient visits A collaborative and supportive interdisciplinary team approach to care The chance to see patients in their home environment, gaining a true understanding of their needs Being part of a company that has been improving people’s lives for over 40 years Must-Have Qualifications To thrive in this role, you’ll need: A current Licensed Practical Nurse (LPN) license in the State of MN, active and in good standing 2 years of experience as an LPN preferred The ability to travel within the geographic area serviced by the office; valid driver’s license, reliable transportation, and proof of auto insurance required CPR Certification Your Responsibilities As an LPN, you will: Participate in ongoing interdisciplinary assessments of patients Collaborate in developing and evaluating care plans with patients and caregivers Provide services as ordered by physicians according to the care plan Recognize and report changes in patients’ conditions to the RN supervisor Educate and counsel patients and families on healthcare needs Complete timely and accurate clinical notes documenting patient progress Ready to Make a Difference?
