Overview

We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will provide comprehensive patient care in the home. This role requires a strong foundation in physiotherapy principles, anatomy, physiology, and medical terminology to develop and implement effective care plans tailored to diverse patient populations such as pediatrics, geriatrics, orthopedics, and those recovering from acute or chronic conditions. The Physical Therapist will play a vital role in discharge planning, patient education, and promoting recovery through evidence-based practices.

Duties

  • Assess patients' physical conditions and develop individualized care plans based on their needs and goals.
  • Implement therapeutic interventions such as trigger point therapy, manual therapy, and exercise programs to improve mobility, strength, and function.
  • Conduct discharge planning to ensure seamless transition from hospital or clinic to home or community settings.
  • Provide patient care for home health setting.
  • Collaborate with multidisciplinary teams to coordinate treatment strategies for complex cases involving orthopedics, geriatrics, or pediatric populations (pediatrics).
  • Maintain accurate documentation using EMR systems to track progress and adjust treatment plans accordingly.
  • Educate patients and caregivers on injury prevention, rehabilitation exercises, and health maintenance strategies.
  • Stay current with advancements in physiotherapy, physiology knowledge, and medical terminology to ensure best practices.
  • Participate in team meetings for case discussions and contribute to quality improvement initiatives related to patient outcomes.

Requirements

  • Valid license as a Licensed Physical Therapist with appropriate state certification.
  • Proven experience in various clinical settings such as acute care, hospital experience, home & community care, or outpatient clinics.
  • Strong knowledge of anatomy, physiology, and relevant medical terminology.
  • Experience with developing and managing comprehensive care plans for diverse patient populations including geriatrics, pediatrics, and orthopedic cases.
  • Proficiency in using EMR systems for documentation and patient management.
  • Familiarity with specialized techniques such as trigger point therapy.
  • Ability to work effectively within multidisciplinary teams focusing on holistic patient recovery.
