Role Description
The Program Coordinator provides leadership and oversight for residential and community-based programs serving youth and adults with developmental disabilities, behavioral needs, or those in foster care. This role ensures high-quality, person-centered services by coordinating treatment planning, documentation, staff performance, and regulatory compliance with the Tennessee Department of Children's Services (DCS), the Tennessee Department of Disability and Aging (DDA), and the Council on Accreditation (COA) standards.
This position is ideal for a mission-driven professional who excels in service coordination, team leadership, crisis response, and detailed case documentation.
Key Responsibilities
Service Coordination & Treatment Planning
- Lead development and implementation of Individualized Treatment Plans (ITPs) and Person-Centered Support Plans.
- Align plans with DCS Permanency Goals and update them based on youth progress.
- Collaborate with clinicians, case managers, families, and stakeholders to ensure coordinated care.
Documentation & Reporting
- Review and approve case notes, progress logs, monthly summaries, and data sheets.
- Submit accurate monthly summaries and incident documentation in Tennessee Family and Child Tracking System (TFACTS) within required timelines.
- Maintain documentation consistent with DCS, DDA, COA, and agency standards.
Youth Assessment, Behavior Support & Stabilization
- Complete intake assessments and ongoing reviews of youth behavior and needs.
- Lead stabilization planning following behavioral incidents or crises.
- Coordinate with therapists, educators, and providers to maintain consistent behavior strategies.
Incident Reporting & Risk Management
- Serve as primary incident coordinator and ensure timely reporting to Family Service Workers.
- Conduct incident debriefings, root-cause analysis, and corrective actions.
- Track incident trends and support agency PQI initiatives.
Staff Leadership & Team Support
- Supervise Direct Support Professionals and House Leads.
- Conduct site visits, performance reviews, and staff coaching.
- Ensure required staff training and competency standards are met.
Compliance, Quality Assurance & Program Operations
- Maintain compliance with DCS, DDA, and COA regulations.
- Conduct chart audits, safety checks, and environmental reviews.
- Support agency audits, accreditation reviews, and improvement efforts.
Family & Stakeholder Collaboration
- Communicate regularly with families, guardians, case managers, and providers.
- Participate in Child & Family Team Meetings and permanency planning.
- Represent the agency professionally in community and stakeholder settings.
Qualifications
- Bachelor's degree in social work, Psychology, Human Services, or related field (master's preferred).
- At least 2 years of experience in Intellectual or Developmental Disabilities (IDD), behavioral health, foster care, or youth-serving programs preferred.
- Knowledge of DCS, DDA, and COA regulations strongly preferred.
- Strong leadership, communication, organization, crisis-management, and documentation skills.
- Valid driver's license, reliable transportation, and ability to travel between program sites.
- Able to pass background checks and drug screening.
