Tice Valley Post Acute is seeking a Licensed Vocational Nurse (LVN) to serve as an MDS Assistant. This is a full-time position with a competitive salary range of $42-$45 per hour, depending on experience.

Responsibilities:

  • Oversee weekly PDPM meetings for skilled patients at the facility.

  • Track and monitor the effective utilization of PDPM tools, including Physician Diagnosis Verification, Admission/Discharge CSR (GG), Weekly CSR, PDPM Weekly Tracking Tool, and Point Right.

  • Monitor QM and QASP performance monthly and quarterly, providing data analysis and clinical recommendations for optimal outcomes.

  • Train new MDS Coordinators on PDPM and MDS processes.

  • Assess and determine the health status and level of care for all new admissions.

  • Ensure the accurate and timely completion of all MDS Assessments, including PPS Medicare, quarterly, annual, and significant change assessments.

  • Communicate the level of care for new residents to all relevant disciplines.

  • Coordinate interdisciplinary participation in completing MDS for new admissions in accordance with regulatory timelines.

  • Ensure documentation completeness and thoroughness as mandated by federal, state, and medical standards.

Certificates, Licenses, Registrations:

  • Proficiency in Microsoft Suite products is required.

  • Knowledge and experience with PCC is preferred.

  • Must maintain all required continuing education and remain in good standing with the State Board of Nursing.
