Duties
- Provide comprehensive assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts and other assistive devices.
- Monitor and observe patients' health status by taking vital signs, recording medical information accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, and reporting any changes promptly.
- Administer medications following prescribed protocols while maintaining strict adherence to HIPAA regulations to ensure patient confidentiality.
- Assist with specialized care needs such as Alzheimer's care, dementia care, memory care, hospice support, catheter care, and medication management.
- Support patients with developmental disabilities and those in long-term or home health settings by providing personalized caregiving tailored to individual needs.
- Prepare nutritious meals and assist with meal planning to promote healthy living and dietary compliance.
- Perform light housekeeping duties including cleaning, laundry, and maintaining a safe environment to enhance overall patient comfort.
- Document all patient interactions and medical records accurately using EHR/EMR systems for seamless communication among healthcare teams.
- Utilize medical terminology appropriately when recording observations or communicating with healthcare professionals.
- Employ patient observation skills to identify potential health issues early and escalate concerns as needed.
- Support hospice and palliative care patients by providing compassionate companionship and ensuring comfort measures are met.
- Operate medical equipment such as blood pressure cuffs, thermometers, pulse oximeters, and other vital signs monitoring tools effectively.
- Assist with transfers and mobility using proper techniques to prevent injury for both patients and caregivers.
- Engage actively with patients to foster trust and promote a positive caregiving experience in home or community settings.
Qualifications
- Proven experience as a Home Health Aide or in similar caregiving roles within assisted living facilities, nursing homes, or home health environments. Long-term care experience is highly valued.
- Strong knowledge of medical terminology, medication administration protocols, and documentation standards compliant with HIPAA regulations.
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care training or Alzheimer's/dementia certification are a plus.
- Demonstrated ability to work effectively with people with disabilities or developmental challenges across diverse age groups.
- Experience operating EMR/EHR systems for accurate medical documentation and record keeping.
- Ability to perform patient observation diligently while maintaining a positive attitude under pressure.
- Skilled in assisting with ADLs using appropriate techniques and equipment like Hoyer lifts or transfer belts.
- Knowledge of infection control practices including cleaning routines necessary for maintaining a safe environment.
- Excellent communication skills to collaborate seamlessly with healthcare teams while respecting patient privacy under HIPAA guidelines.
- Valid driver's license preferred if travel between multiple client locations is required; reliable transportation is essential.
