Wheeler Creek CDC is dedicated to supporting lower-income residents by providing the resources and networks vital for progression from dependency to self-sufficiency. Through our three main programs — Homeless Prevention, Permanent Supportive Housing, and Rapid Re-Housing — we help individuals and families obtain and maintain stable housing, improve their quality of life, and prevent homelessness recidivism.

Position Overview

We are seeking Program Case Managers to provide intensive, housing-focused case management services for individuals and families experiencing or at risk of homelessness. Case Managers will be assigned to one of the three programs but should possess the adaptability and skills to support clients across programs through cross-training and internal transfers as needed.

Under the supervision of the Program Manager and Case Manager Supervisor, Program Case Managers will work in the community and in office settings to help clients secure housing, comply with lease obligations, and achieve long-term stability and self-sufficiency.

Key Responsibilities

(Duties vary depending on program assignment)

  • Assessment & Planning
  • Assess individuals and/or families for homelessness risk and housing stability needs.
  • Develop, implement, and monitor Individualized Service Plans / Family Goal Plans with both short- and long-term goals.
  • Ensure clients comply with lease agreements and progress toward self-sufficiency.
  • Client Support & Advocacy
  • Meet regularly with clients to review progress and provide follow-up support.
  • Conduct home visits and field-based meetings throughout Washington, DC.
  • Assist with housing search and placement, including landlord engagement, advocacy, and mediation.
  • Provide diversion, mediation, housing counseling, and financial assistance when appropriate.
  • Work with families to resolve issues related to lease compliance or program rules in collaboration with property management.
  • Referrals & Service Linkage
  • Connect clients with supportive services such as employment, legal aid, mental health care, substance use treatment, and financial counseling.
  • Build and maintain partnerships with community resources to enhance client support.
  • Documentation & Compliance
  • Maintain accurate and timely records in HMIS, Quick Base (OCTO), and paper case files in accordance with funder and agency requirements.
  • Ensure client files meet program regulations.
  • Participate in quarterly Case Review meetings, staff meetings, and trainings.
  • General
  • Advocate on behalf of clients to ensure fair housing and equitable treatment.
  • Perform other duties as required to meet program and agency objectives.

Qualifications

  • Required
  • Bachelor's degree in Social Work, Psychology, Sociology, Counseling, Human Services, or a related field.
  • OR certification/licensure (e.g., Certified Addictions Counselor) may substitute for degree.
  • At least 2 years of experience providing case management or direct social services.
  • Strong interpersonal, problem-solving, and conflict resolution skills.
  • Excellent written and verbal communication.
  • Ability to conduct home visits and travel across Washington, DC (75% field-based).
  • Proficiency with case management systems (HMIS, Quick Base, or similar).
  • Must pass a background check.
  • Reliable transportation required.
  • Preferred
  • LGSW credential.
  • Bilingual (Spanish/English or other languages).
  • Experience working with families, homelessness, mental health, or substance use.
  • Knowledge of Housing First and strengths-based, client-centered approaches.

Job Type: Full-time

Pay: $47, $51,700.00 per year
