Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in providing support and guidance to individuals facing various challenge off victimization. This position requires a strong commitment to crisis intervention and management, as well as the ability to work collaboratively with clients to develop effective treatment plans and support systems.
Responsibilities
- Conduct comprehensive assessments of clients' needs and develop individualized service plans.
- Provide crisis intervention and management to clients in urgent situations.
- Facilitate access to rescources.
- Mentor clients in workforce development initiatives to enhance their employability skills.
- Collaborate with community organizations, social services, and healthcare providers to coordinate care for clients.
- Maintain accurate documentation of client interactions, progress notes, and case files in compliance with agency policies.
- Experience
- A background in social work or a related field is preferred, with experience in crisis intervention and management.
- Familiarity with motivational interviewing techniques is highly desirable
