Overview
We are seeking a dedicated and knowledgeable Nutritionist to join our healthcare team. This is a hybrid position that either full time/part time, with as little as 1 day a month on site. The ideal candidate will possess a strong background in dietetics, physiology, and patient assessment, with experience in various care settings including long-term care, acute care, and hospital environments. This role involves developing personalized nutrition plans, providing health coaching, and collaborating with multidisciplinary teams to improve patient outcomes. The Nutritionist will play a vital role in managing care plans for diverse populations, including geriatrics and patients requiring specialized nutrition such as tube feeding and weight management. A commitment to evidence-based practice and proficiency in EMR systems are essential for success in this position.
Responsibilities
- Conduct comprehensive patient assessments to determine nutritional needs and develop individualized care plans.
- Provide expert guidance on nutrition, dietetics, and weight loss strategies.
- Monitor patient progress and adjust nutrition interventions accordingly, ensuring optimal health outcomes.
- Collaborate with healthcare professionals to coordinate nutritional care within multidisciplinary teams across hospital, long-term care, and acute care settings.
- Educate patients and caregivers on dietary modifications, lifestyle changes, and health coaching techniques to promote wellness.
- Supervise dietary department staff when applicable, ensuring compliance with clinical standards and safety protocols.
- Document all assessments and interventions accurately within EMR systems for ongoing patient management.
- Stay current with the latest research in nutrition science, geriatrics, and diagnostic evaluation to inform clinical practice
Requirements:
- Active Maryland (MD) license in the trained profession - Required.
- Minimum of 6 months of experience , at least 900 hours experience
- Academic Requirements - For Licensure as a Nutritionist (CNS) A master's or doctoral degree from an accredited college or university in nutrition sciences with (emphasis in human), food and nutrition, dietetics, community nutrition, public health nutrition, or equivalent academic training such as nutrition education and food science.
- Demonstrated ability to work collaboratively within cross-functional teams.
- Excellent communication skills, both verbal and written, with the ability to effectively communicate with all levels of the organization.
- Reliable transportation for clinic visits.
- Proficiency in computer/iPad technology for documentation and communication purposes.
Experience
- Proven experience in nutrition counseling for more than 6 months, or 900 hours
- Strong knowledge of physiology, dietetics, and diagnostic evaluation methods related to nutrition therapy.
- Familiarity with EMR systems used and telehealth in healthcare documentation is required.
