About the Role

Spirit Homecare & Hospice is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our growing home health team. This role is ideal for nurses who value meaningful patient interactions, hands-on care, and the opportunity to make a difference in the comfort of patients' homes. As an integral member of our interdisciplinary team, you will collaborate closely with patients, families, and healthcare professionals to deliver high-quality, patient-centered care.

Compensation & Benefits

Per-Visit Pay: $28 per completed visit (minimum 35 minutes)

Flexibility: Choose from full caseloads or PRN visits based on your schedule

Additional Earnings: Optional on-call opportunities available

Team Collaboration: Work alongside RNs, PTs, OTs, and physicians in a supportive environment

Professional Development: Comprehensive orientation, mentorship, and ongoing training

Key Responsibilities

  • Provide direct patient care in accordance with physician orders and established care plans
  • Administer medications, wound care, and prescribed treatments safely and accurately
  • Document patient observations promptly and report any changes to the RN Case Manager or physician
  • Educate patients and caregivers on condition management, medications, and safe care practices
  • Maintain timely, accurate, and compliant documentation in the EMR system
  • Assist with specialized nursing procedures under RN supervision
  • Adhere to agency policies, procedures, and all state and federal regulations

Qualifications

  • Active Ohio LPN license (or eligibility for licensure)
  • Graduate of an accredited Practical Nursing program
  • Minimum of 1 year of nursing experience preferred (home health experience a plus)
  • Knowledge of Medicare guidelines and CMS regulations preferred
  • Proficiency with electronic documentation systems
  • Current CPR certification
  • Reliable transportation, valid driver's license, and proof of auto insurance

Work Expectations

  • Territory: West Chester Township and surrounding communities
  • Care Settings: Patient homes, assisted living facilities, and related environments
  • Physical Requirements: Ability to walk, bend, lift, and stand for extended periods
  • On-Call: Optional evening, weekend, and holiday rotations
  • Communication: Maintain schedule and availability updates with the Director of Nursing
