Online Lifestyle Coach & Targeted Case Manager (Remote, Part-Time)

Kentucky Counseling Center

Kentucky Counseling Center is seeking a compassionate, goal-oriented professional to serve a dual role as an Online Lifestyle Coach and Targeted Case Manager. This unique position supports client growth through non-clinical life coaching as well as Medicaid-based case management, helping individuals improve daily habits, access resources, and achieve sustainable personal progress. This role is fully remote and focused on empowering clients through personal development, resource navigation, and consistent support.

Key Responsibilities

Lifestyle Coaching (Non-Clinical)

  • Guide clients through personal or professional transitions
  • Support goal-setting, motivation, and habit development
  • Improve work–life balance, confidence, communication, and relationships
  • Assist with stress management and accountability
  • Encourage personal growth and ongoing self-improvement

Targeted Case Management (Medicaid Clients)

  • Work one-on-one with clients to develop and achieve person-centered goals
  • Assist with financial, community, and mental health resources
  • Coordinate care with providers and ensure follow-through
  • Provide education, advocacy, and consistent support
  • Meet a minimum of 13 billable services per month in accordance with Medicaid and KCC standards

Collaboration & Documentation

  • Work alongside a multidisciplinary team of service providers
  • Maintain accurate, timely documentation
  • Utilize telehealth platforms for all services

Requirements

  • Experience in life coaching, case management, or client-focused support roles
  • BA/BS in a Human Services field required; MA/MS in Human Services or Social Work preferred
  • Prior experience in behavioral health, mental health, or co-occurring populations for TCM services
  • Strong motivational, communication, and interpersonal skills
  • Organized, self-directed, and comfortable managing a remote caseload
  • Completion of Medicaid TCM core training (12 hours), plus SED, SUD, and CCPCH trainings within 90 days of hire
  • Genuine passion for helping others grow and succeed

Schedule & Job Type

  • Part-Time, Remote
  • Flexible schedule (Monday–Friday, 8-hour shift flexibility)
