Location: 2330 N Loop 1604 West, San Antonio, Texas (ON-SITE)
Salary: $43.00/hour (Non-Exempt)
Academic Req: Required – Registered nurse license, Bachelor of Science in Nursing
Certifications: Good standing as a licensed Registered Nurse with the State of Texas Board of Nursing; current BLS certifications (PALS preferred), First aid, CPR, Emergency behavior intervention
Work experience: Required – At least 5 years of experience as a RN; Preferred – pediatric nursing experience
Schedule: Open Shifts
Sunday - Thursday: 11 PM to 7 AM
Tuesday - Saturday: 3 PM to 11 PM, and 11 PM to 7 AM.
Critical Action Items & Measurable Deliverables:
1.Meet and maintain all federal and state regulatory guidelines and standards applicable to this position.
2. Provide telehealth triage services to assess patient needs and evaluate symptoms. This includes answering patient inquiries, providing patient teaching, and determining the urgency and appropriate course of action.
3. Conduct in-person and virtual assessments based on established protocols and accurately document medical findings.
4. Gather, collect, and clarify patients' current condition and past medical history.
5. Effectively communicate with Medical Director as needed for reporting findings which are concerning for patient's health and well-being.
6. Communicate with RN Managers and other medical staff to devise individual plans of care and acts as a nurse support to other medical staff.
7. Implement Compass Connections safety protocols including evacuating with children and other staff in case of an emergency.
8. Maintain confidentiality of health care records and information in keeping with HIPAA and Compass Connections confidentiality and Personally Identifiable Information (PII) protocol and procedures.
9. Address other job duties as assigned or for unforeseen matters that require attention to maintain a fluid and fully functional program.
Other Responsibilities:
1. Plan, organize and, prioritize daily work using sound judgement.
2. Monitor established health care plans for service population at the sites within the timeframe established by our State and Federal partners.
3. Ensure accurate documentation in accordance with agency policies and procedures, regulatory requirements and standards, and program timeline requirements.
4. Meet all program and federal partners' documentation and processes requirements.
5. Other duties as assigned.
Requirements:
1. Pass a pre-employment drug screen and random drug screens throughout employment.
2. Provide proof of work eligibility status upon request.
3. Pass a pre-employment and biennial criminal background checks.
4. Demonstrate the ability to:
a. Work collaboratively with other staff members, service providers and professionals.
b. Communicate effectively in writing and verbally in English.
c. Work in a fast-paced environment and make decisions based on the needs of the program and service population, while maintaining emotional control and professional composure at all times.
d. Maintain computer literacy required to meet the responsibilities of the position.
e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
5. Demonstrate a working knowledge of all Compass Connections policies and procedures.
6. Must provide current physical-medical release and provide proof of immunizations, titers, or declinations for the following:
a. MMR
b. Varicella
c. Hep B
d. Hep A
e. Tetanus
f. PPD
g. Meningococcal (preferred)
7. Must be able to perform physical activity such as extensive walking, stretching, and bending.
8. Must be able to work in a team environment.
9. Must be able to maintain professional people skills, including oral, written, listening, and non-verbal communication skills.
