What You'll Do

  • Welcomes and connects new residents to the community to build rapport and introduce available community and external services, including health care and support groups.
  • Educates new residents on community safety measures including pendants and apartment motion sensors.
  • Collaborates with residents to assess, plan, implement, and document support needs, coordinating resources from within the community and external partners (e.g., home care, home health, hospice, mental health services, and case management firms)
  • Ensures key documents [e.g., Vial of Life, Physician Orders for Life Sustaining Treatment (POLST), Durable Power of Attorney (DPOA) and Personal Representative/ Executor information) are accurate, up to date, and properly stored in master database.
  • Collaborates with cross-functional teams, including resident committees, and facilitates regular cross-departmental team meetings to support resident well-being and safety.
  • Assures prudent utilization of identified resources by evaluating the options available and balancing cost and quality to assure optimal well-being and financial outcomes.
  • Advises residents and families on issues of transition and end of life choices, ethical concerns, powers of attorney, and guardianship. Serves as a resource and referral for complex, technical, or legal questions.
  • Respects, honors, and supports resident end of life choices, including Death with Dignity or Voluntarily Stopping Eating and Drinking.
  • Manages the continuum of care with KPI and OKR metrics and creates monthly reports.
  • Works with prospective Assisted Living (AL) resident/DPOA, and AL Sales and Outreach Manager to coordinate financial screen and admission process for transition to AL.
  • Supports transitions from Independent Living (IL) to Assisted Living and works with the AL team to ensure a smooth transition.
  • Collaborates with the AL team for transitions of short-term stay residents back to Independent Living (IL) to ensure residents have applicable support services in place and can meet the Conditions of Occupancy.
  • Provides input to AL resident assessments and collaborates with interdepartmental teams on resident and program needs.
  • Collaborates with the Sales department on referrals and outreach opportunities.
  • Networks and fosters relationships with outside educational and resource entities that relate to issues of aging, mental health, and allied services.
  • Prepares and presents topics of importance to residents and staff.
  • Responsible for supporting Horizon House's Continuum of Care model and utilizing the Conditions of Occupancy to manage the continuum of care.
  • Establishes relationships with hospital and skilled nursing facility partner discharge planners and follows up on residents in these settings to confirm discharge plans and alignment with Conditions of Occupancy for a return to IL or transition back to AL.

The Ideal Candidate will possess the following qualifications:

  • Bachelor's degree in Social Work from an accredited institution with direct and recent Geriatric experience OR a minimum of five years' recent experience as a Certified Geriatric Case Manager.
  • Minimum three (3) years' experience in serving older adults in a community-based setting is required.
  • Understanding and demonstrated experience in working with older adults, dementia, aging issues, complex family dynamics and transition plans.
  • Familiarity with Assisted Living and interdisciplinary team approach, knowledge of community services and programs. (preferred)
  • Working knowledge of Window's based computer systems and Microsoft office products.
