The Role:

The Support Coordinator is responsible for assisting clients (Community Members) as they implement their annual goals and objectives. Support Coordinator provides advocacy & problem solving, case management, and life skills instruction. Support Coordinators enable Community Members to achieve and maintain an independent lifestyle in accordance with their capabilities and in tandem with the larger team.

Essential Responsibilities:

  • Plan, prepare for and schedule annual reviews. Assist community member in drafting annual individualized care plan and goals based on SC's annual assessment, reports from other providers and the community member's personal goals. Assist client with any changes or new objectives created as a result of the annual team meeting.
  • Meet with Community Member for the agreed upon number of hours per week/month and establish the schedule noting that consistency is necessary. Maintain regular and consistent communication with family, team members and outside professionals as determined by need and/or the team.
  • Impart good nutrition skills through menu planning, creating shopping lists, making healthy choices, proper food storage, meal preparation and kitchen safety.
  • Practice sound money management skills with each individual on your caseload, which may include-but not limited to-establishing a weekly budget, banking (in-person or online), timely bill payments, accessing additional funding and/or supplemental funding streams based on everyone's financial profile.
  • Assist with home maintenance skills and home safety. Monitor maintenance skills and the hiring of a cleaning service, creation of a chore checklist, hand-over-hand assistance).
  • Review proper hygiene and assist in developing a plan when warranted. This may include hiring a home health organization.
  • Coordinate health maintenance and care with individuals, their guardians, physicians, dentists, therapists, home health agencies, etc.
  • Provide transportation of clients as needed; use of personal vehicle may be required.
  • Support strong interpersonal communication strategies as indicated for each individual on caseload. Assist CM in creating a strong social network and in maintaining healthy relationships with family, roommates, friends and colleagues.
  • Provide advocacy on community members' behalf to ensure timely, fair and quality service. Enlist help from supervisor as needed.
  • Utilize Setworks to properly collect and maintain accurate and timely case records, relief notes and other data as required by supervisor.
  • Maintain shared work schedule with supervisor in Outlook Calendar, Setworks or EVV. Submit relief notes to supervisor one week prior to PTO. Prepare case notes, reports and other administrative requirements as outlined by policy.

MINIMUM EDUCATION REQUIREMENTS:

Bachelor's Degree in relevant field preferred in tandem with two years' experience working with adults with cognitive/social disabilities.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

  • Cultural sensitivity and proven ability to work with diverse populations.
  • Excellent oral and written communication.
  • Computer literacy; Outlook, Excel, Word
  • Excellent organizational skills and proven ability to meet deadlines and arrive on time to appointments
  • Valid CT driver's license
