As a Licensed Vocational Nurse (LVN), you will play a crucial role in delivering compassionate and high-quality patient care across in a home health settings. Your expertise will be essential in supporting patients' health and well-being, whether in long-term care facilities, hospitals, or outpatient clinics. What you'll do
- Provide direct patient care, including administering medications and treatments as prescribed.
- Conduct thorough patient assessments and monitor vital signs to ensure optimal health outcomes.
- Assist with medical procedures such as IV insertion, catheterization, and wound care.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Educate patients and their families on health management, medication administration, and post-care instructions.
- Maintain accurate medical documentation in compliance with HIPAA regulations and facility policies.
- Support patients with developmental disabilities, Alzheimer's care, and dementia care by providing tailored assistance.
- Participate in discharge planning and case management to facilitate smooth transitions of care.
Basic qualifications
- Valid LVN license in the state of practice.
- Strong knowledge of medical terminology and nursing practices.
Preferred qualifications
- Experience with Epic or other Electronic Health Record (EHR) systems.
- Critical care experience in settings such as ICU or emergency medicine.
- Familiarity with ICD-9 and ICD-10 coding for accurate medical documentation.
- Background in geriatrics or home health nursing.
- Proficiency in phlebotomy and specimen collection/processing.
Pay: $ $50.00 per hour
