We are currently seeking full-time and part-time staff to pick our residents up from their daytime activities in the afternoon, drive them home, and assist them to achieve their goals working toward independent meal preparation, home maintenance, and self care in the evenings. Shifts are available 2:15-10:15pm weekdays and and all day on weekends, including overnight hours. Job Summary: As a member of the Rising Phoenix Community Living, LLC team, the Direct Care Staff assists to inform, implement, and follow through with each consumer's Person-Centered Support Plan (PCSP). The Direct Care Staff provides aid, support, training, and supervision to persons served. Staff maintains appropriate records and ensures that documentation is completed and submitted as required. Staff will support the direction and leadership of Rising Phoenix Community Living, LLC and its day and residential programs.

Responsibilities:

· To develop, implement, and follow through with the PCSP:

  1. Assist in developing goals and objectives that encourage the person served to become more independent.

  2. Attend and participate in PCSP and Team Meetings.

  3. Implement activities, services, and support plans as documented and approved by the team.

  4. Follow through with PCSP, services, supports and activities by completing required documentation and communicating progress to team members.

· Provide assistance, support, training, and supervision to persons served:

  1. This includes but is not limited to assisting, supporting, training, and supervising as determined by the individual functioning level of all persons served and/or their individual desire. In cases where the functioning level is low, the Direct Care staff is 100% responsible for these items:

a. Health and personal hygiene- Monitoring and assisting with diet/nutrition, following approved diets, completing menus, dressing, teeth brushing, bathing, haircuts, hair combing, nail grooming, toileting, taking of temperature, blood sugar levels, blood pressure, etc.

b. Medication Administration- Train with the Rising Phoenix Community Living, LLC nurse to assist in administering medication to consumers. Administer medications to consumers as needed. Immediately notify Nurse or Immediate supervisor of all medication related questions or concerns. Maintain Medication Administration Certification.

c. Home management- meal planning, shopping, cleaning, decorating, communicating needed maintenance and repairs, following doctor ordered diets, etc.

d. Socialization- Assisting with communications via telephone, community, friends & strangers

e. Community awareness and inclusion- Encouraging a wide variety of activities for persons served

f. Transportation- Driving clients and pumping gas for company vehicles

g. Emergency preparedness- tornado, fire, 911, emergency information, power outage, medical emergency, ANE procedures

h. Self-advocacy- Assist client with making good choices by offering options, explaining consequences of decisions, etc.

i. Daily scheduling

j. Rights and Responsibilities- monthly training and advocacy

k. All other services/plans as outlined and approved in PCSP.

· Maintain appropriate records and ensure documentation is completed and submitted as required.

  1. Maintain up-to-date, accurate, and secure working and resource files on each person served per Rising Phoenix Community Living, LLC policy.

  2. Follow confidentiality and HIPPA procedures for working and resource files of person served and personal information.

  3. Complete all reports and data related to the PCSP as well as any other required documentation and submit to supervisor by the first working day of each month.

  4. Complete all reports and data related to attendance and expenditures and submit to supervisor by the first working day of the month.

  5. Complete all reports and data related to Health and Safety submit by the first working day of the month.

  6. Reports related to Health and Safety to be submitted within 24 hours include but are not limited to:

a. ANE reports

b. Seizure reports

c. Incident reports

d. Accident reports

e. Medication errors

f. Maintenance requests and concerns.

· Attend required in-service sessions and maintain required certifications without lapse.

  1. Complete and maintain required First Aid, CPR, CPI, and Medications certifications training.

  2. Attend and participate in other trainings as assigned by immediate supervisor or directors.

· Support the direction and leadership of Rising Phoenix Community Living, LLC and its residential program.

  1. Follow all Rising Phoenix Community Living, LLC Company and Departmental policies and procedures.

a. Accurately and consistently complete and submit time sheets.

b. Communicate overtime requests to immediate supervisor/directors.

c. Communicate needed time off within appropriate time lines. 4 hours before scheduled shift for sick leave/call-ins and 2 weeks earlier for vacation requests.

d. Communicate after hour emergencies that affect the health, welfare, and/or safety of clients or staff to immediate supervisor or on-call person.

e. Perform other duties related to this position as requested.

f. Essential Functions:

v. Discuss issues and problems with others and encourage the efforts of others

vi. Awake supervision

vii. Keep certifications current without lapse.

viii. Maintain a current valid driver's license and insurance on your personal vehicle

  1. Maintain professional and open lines of communication with all team members, including family and guardians, within HIPPA and Rising Phoenix Community Living, LLC confidentiality guidelines.

  2. Participate in meetings.

  3. Maintain professionalism always when representing Rising Phoenix Community Living, LLC.

  4. Maintain a good working relationship with your immediate supervisor and directors of RPCL.
