Description

DEFINITION:

Under immediate to general supervision, provides targeted support services to provide a system of care that is child-centered with the needs of the child and family dictating the types and mix of services provided to assist in preventing an out-of-home and out-of-school placement. The Department Head for this position is the Chief Clinical Officer. This position is immediately supervised by the Clinical Administrator and/or assigned Team Lead.

EDUCATION AND EXPERIENCE:

High School Diploma or equivalent. Experience in seeking and accessing state and community resources and a willingness to help families access resources.

Experience raising a child with behavioral/emotional issues.

Experience accessing services for children with behavioral/emotional issues and navigating the child serving system.

An expectation of core competencies in relation to individuals with co-occurring disorders.

KNOWLEDGE AND SKILLS:

Ability to evaluate situations and adopt an effective course of action, to delegate responsibilities, organize and present facts effectively.

Ability to establish working relationships with clinical, administrative, and support staff in order to develop professionalism and coordination within the team.

Able to establish and maintain an effective working relationship with fellow employees, clients and the public.

Ability to interact with staff, clients and administration and the community both in person and on the telephone.

Ability to maintain strict confidentiality guidelines for interactions regarding client information.

Ability to work well under pressure meeting deadlines with efficiency and consistency.

Such specialized training and/or certification in mental health services or related field as may be required in order to provide adjunctive services, i.e., Systems of Care/Wraparound training.

Personality and mature presentation of oneself demonstrating the ability to work with various systems while maintaining objectivity and cooperative attitude.

JOB DUTIES AND RESPONSIBILITIES:

Responsible and immediately supervised by the assigned Team Lead:

• Assist families in developing and recruiting informal supports to be a part of the Family Team meetings.

• Complete (or co-complete) Strengths Needs Cultural Discovery (SNCD) assessments, Functional Assessments/Crisis Plans, Safety Plans, and Care Plans/Updates.

• Provide support for families with the eventual goal of the family utilizing their natural supports.

• Facilitate or co-facilitate Family Team meetings and ensure informal supports are included and part of the team.

• Implement the Do For, Do With, and Cheer On model for families.

• Maintain close communication with the Team Lead and other clinical and administrative staff in order to ensure the highest quality services are delivered for youth and families.

• Complete flex-fund requests when necessary.

• Monitor and evaluate services provided to the families. Contact families as needed to monitor progress and inquire about new needs.

• Personality and mature presentation of oneself, demonstrating the ability to work with various systems while maintaining objectivity and cooperative attitude.

• Regular and predictable attendance is an essential job requirement.

• Employee must be willing and able to perform all job-related travel normally associated with this position (this may include transportation of clients).

• Participates in required staff meetings.

• Performs other reasonably related duties as assigned by the immediate supervisor or other management as requested.

• Quality of work will be evaluated through training, skill, or special purpose. Must be competent and adequate. Must meet "satisfactory" expectations at any scheduled job performance evaluation.

Must perform the specific job duties as listed above to meet position expectations.

In addition, must possess skill in working cooperatively and effectively with clients, staff, management and other professional groups; in exercising mature judgement in dealing with people; in presenting ideas clearly and accurately; in reading and comprehending the English language; and in communicating effectively, both orally and in writing.
