Hospice (RN) Case Manager - Float

This float RN position is designed for experienced hospice nurses who value flexibility and variety, offering the chance to support patients across multiple locations while making a meaningful impact during every visit.

Salary and Benefits
  • Starting Salary: $85,000
  • Health Insurance: Employer covers 70%
  • Dental and Vision Insurance: Provided
  • Paid Time Off: 8 days in the first year
  • Paid Holidays: 6
  • Mileage Reimbursement: $0.50 per mile
  • Cellphone Allowance: $60 per month
  • Short-Term Disability: Available
  • Life Insurance: Paid by employer
  • Retirement Package: 401(k) with match
  • Full Support Staff
Responsibilities
  • Monday - Friday schedule, 8:30 AM - 5:00 PM
  • Float across multiple service areas based on patient and staffing needs
  • Perform patient admissions, routine visits, and symptom management
  • Deliver compassionate, high-quality hospice care in patients' homes
  • Assess patient conditions and update care plans accordingly
  • Collaborate with physicians and interdisciplinary team members
  • Educate patients and families on hospice services and symptom management
  • Maintain accurate documentation and ensure timely communication
  • Travel daily within the metro Atlanta area
Requirements
  • Clear and active RN license in the state of Georgia
  • Minimum of 1 year of hospice experience required
  • Excellent clinical and communication skills
  • Valid driver's license, reliable transportation, and current auto insurance
  • Current CPR certification
  • Flexibility to cover varying territories day-to-day
