Department, Salary, Location, and Schedule

  • Department: ISH; 2 positions available
  • Salary: $44,000 $45,000
  • Location: Union, NJ (based)
  • Work hours: 40 hours weekly with various shift options (see description for details).

Position Overview

  • As part of a multi-disciplinary core services team, provides wellness assessment and direct services to people with serious mental illness enrolled in the PACT Program.
  • Teach skills to support individualized recovery journeys and community integration.
  • Assist with securing safe, clean, affordable housing, food and clothing, medical and dental services, and financial support.
  • Linkage to entitlement benefits (Medicaid/NJ FamilyCare, Social Security, rental assistance, etc.) and other public assistance including transportation and legal advocacy.
  • Observe and assess service recipients concerns, symptoms, and behavior; monitor medication side effects during self-administration and face-to-face contacts.
  • Provide medication education for individuals and families with consent.
  • Coordinate services with community mental health and other medical providers.
  • Provide training and instruction to individuals at home and in the community on personal hygiene, household tasks, money management, transportation, and housing.
  • Deliver wellness services including education on nutrition, exercise, stress reduction, and other wellness tools.
  • Respond rapidly to crises (e.g., accompanying to screening or emergency centers, staying with the person during assessment).
  • Utilize Illness Management and Recovery (IMR) and develop WRAP and Psychiatric Advance Directives; apply Motivational Interviewing, CBT, and related strategies; provide 24-hour rotating on-call coverage.
  • Use evidence-based practices (e.g., motivational interviewing, IDDT, CBT, IMR, Trauma-Informed Care, PSH, Career Development) when engaging individuals in recovery.
  • Provide technical assistance and education to team members on recovery and wellness; develop teaching materials (e.g., Wellness Recovery Action Plans) with the team.
  • Support individuals in finding and maintaining employment and/or education/training.
  • Support the service recipients family and social network to manage illness-related stress and promote wellness; provide education to enable positive, supportive interactions.

Education / Experience

  • Masters degree with one year of MH services experience, or
  • Bachelors degree in a behavioral health science with two years MH services experience, or
  • Associates degree in Psychiatric Rehabilitation with two years MH services experience, or
  • Demonstrated volunteer or paid experience with individuals with serious mental illness may substitute for a Bachelors degree
  • Valid drivers license required
  • No more than one moving violation in the past 12 months
  • Vehicle required
