SOCIAL WORKER/CASE MANAGER

Position Title: Social Worker/Case Manager- Full time
Reports to: Clinical Director
General Description: Looking for a case manager to join our team to support Pathways clients in their recovery from chronic and severe mental illness. The case manager role will support a combination of clients both living in our housing and community-based programs with independent living skills, referrals and individualized needs. Our ideal candidate is flexible, creative, compassionate, and adheres to a person-centered approach. We are looking for a skilled case manager that works well independently that is able to document accurately and timely in our Electronic Health Record System. Experience working with clients with serious and persistent mental illness and living in their own apartments, with families, or in residential settings preferred, but not required.
Specific Responsibilities:
1. Ability to assess clients’ strengths, needs, abilities to formulate in conjunction with client, supervisor and community support team a client centered goal/treatment plan.
2. Prepare quarterly treatment plans for clients including assessment of functioning in the following areas: residential, psychiatric/mental, social/vocational, financial and money management, use of community resources, movement toward goals and other relevant information.
3. Provide individual counseling and referrals for group/family counseling as needed.
4. Collaborate with community resources and referral sources to coordinate additional needed services for clients.
5. Provide assistance or arrange for assistance with any or all of the following:
a.) IADL skills (i.e. money management, shopping, housekeeping, personal hygiene, use of leisure time, travel training, etc.)
b.) Obtaining or managing federal, state and/or local welfare benefits.
c.) Making, keeping and following up on medical and/or psychiatric appointments.
d.) Mediation of interpersonal problems with landlords, relatives, etc.
e.) Maintain proper documentation of medication changes for residential clients and communicate this change to residential staff.
6. Maintain client records in our electronic health record system; including complete progress notes, treatment plans, discharge summaries, and other related documents
7. Complete state-required documents – DDAP, Mental Health Waiver, Time Studies, etc. as required
8. Complete CARF required tasks such as quarterly chart reviews of another program, monthly CARF trainings and other specific CARF forms.
9. Participate in clinical meetings, staff meetings and other appropriate community meetings related to resident’s needs and service delivery to ensure that there are no gaps in services for each resident. Includes investigating case situations and presenting information to members of the clinical team which is pertinent to treatment and goal planning.
10. Provide orientation for new clients and staff regarding program philosophy and expectations and transitional services for clients transferring to new case management programs.
11. Crisis intervention as appropriate or needed, verbal de-escalation.
12. Strive to increase professional growth and effectiveness through training and by keeping abreast of current trends and theories in relevant areas.
13. Maintains MSW, LMSW, LCSW state license or strives to fulfill requirements to receive such licensing including weekly supervision and necessary tests.
14. Special projects and other duties as assigned.
Desirable knowledge, skills and abilities:
1. Considerable knowledge of principles of human development and psychopathology, plus case management and social work services delivery.
2. Highly motivated, creative problem solver.
3. Projects a professional demeanor, boundaries, and ethical decision making, aligning with the Social Work Code of Ethics.
4. Understanding and empathy for the needs of the seriously mentally ill.
5. Excellent organizational and writing skills, including writing and oral skills, and a strong team player.
6. Strong knowledge of community resources and state, local and federal entitlement programs for the mentally ill.
7. Computer skills needed for Office 365 and some state of CT software, and ability to learn and use our Electronic Health Record System.
8. MSW, LMSW, LCSW, LPC, LMHC, or similar advanced degree preferred
9. Interest and/or experience in working with adults with severe mental illness and some with dual diagnosis
Qualifications:
1. Minimum requirement of a master's degree in a mental health field with practice experience.
2. Two years’ experience working with people with mental illness preferred
3. Demonstrated ability to communicate effectively both orally and in writing, strong interpersonal skills and time management.
4. Valid driver’s license and use of organization car during normal business hours. Must have a clean driving record.
