Care Coordinator Social Worker Compensation

  • The pay for this position is $60.00.00
  • Benefits are available to full-time employees after 90 days of employment
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

 Care Coordinator Social Worker Highlights

·         Hours are Monday-Friday 8am-4pm. Weekends and on call are not required but availability may be an option.

·         There will be floating within the department

·         60% - Patient Assessment, Referrals, Intervention

·         20% - Documentation

·         10% - Meetings and Program Development

·         10% - Collaboration with Inside/Outside Agencies

Care Coordinator Social Worker Responsibilities

·         Conduct comprehensive psychosocial assessments and screenings for social determinants of health (SDOH), developing tailored care plans that align with patient goals and needs.

·         Support high-risk patients in overcoming barriers to care and navigating the healthcare system, enhancing access to medical homes and social support.

·         Utilize evidence-based interventions to address a range of issues, including behavioral health concerns, financial stress, and housing instability.

·         Maintain accurate documentation of assessments, interventions, and patient interactions per departmental policies and EMR standards.

·         Collaborate with interdisciplinary teams and community agencies to address the evolving needs of patients and ensure comprehensive care.

·         Provide telephonic and virtual counseling to empower patients in self-management and coping strategies for improved health outcomes.

·         Stay informed about community resources, advocating for patient access to social, financial, and insurance systems.

·         Participate in team meetings, training, and professional development opportunities to enhance service delivery and program effectiveness.

·         Support community health workers through education and case oversight to strengthen community engagement.

·         Participate in at least three educational programs annually to stay current in the field.

Care Coordinator Social Worker Requirements

·         3-5 years in a health-related field; hospital experience preferred.

·         Strong understanding of community resources, social services, and insurance systems; familiarity with Epic is a plus.

·         Exceptional organizational and time management abilities; strong clinical, interpersonal, and analytical skills; bilingual proficiency is a plus.

·         Master’s Degree in Social Work from an accredited program and current LSW license in New Jersey.
