ESSENTIAL JOB FUNCTIONS:

The Non-Medical Case Manager, under the direct supervision of the Program Director, promotes the independence, stability, and health of persons living with HIV/AIDS by providing non-medical case management services and eligibility certification to receive services.

Essential Functions of the job include but are not limited to, traveling, driving, having reliable transportation to meet clients, utilizing a computer for typing and conducting research, attending meetings, conducting assessments, and counseling.

 

MAJOR AREAS OF RESPONSIBILITY:

  • Advance health equity by expanding ACCEPT's capacity and services.
  • Complete Eligibility for clients twice a year, in their birthday month and 6 months after.
  • Provide high-quality case management for people who are living with HIV/AIDS (PLWHAs) HIV/AIDS, with a focus on improved health outcomes, housing stability, employment, and income attainment.
  • Be the first point of contact to begin the assessment and intake processes.
  • Conduct comprehensive assessments to address clients' needs and concerns.
  • Coordinate with Community Health Worker II to reassess clients bi-annually, enter client data into databases.
  • Contact clients for engagement/re-engagement in case management and psychosocial care; Schedule and facilitate case management meetings with clients.
  • Assist with developing and updating community partner referral systems.
  • Assist with transportation tracking related to case management and referral programs,
  • Recruit new clients, and enter data from the case management and referral programs into databases.
  • Be the main point of contact for case management, represent case management program in the community, and assist with monitoring overall case management processes.
