Victor health associates is seeking a dedicated Adult or Family Nurse Practitioner either full-time OR part-time, to join our dynamic primary care team. As a Nurse Practitioner in our thriving Private Practice, you will play a crucial role in delivering high-quality patient care, fostering patient relationships, and contributing to the overall success of our healthcare team.
Practice operating hours: Monday - Friday 8:00 AM to 5:00 PM. No weekends or holidays.
The position hours can be adjusted to either full-time or part-time. Part-time would have a fixed 3-day schedule. Patients are seen between 8:00 AM and 5:00 PM. Please include whether you are seeking full or part time employment in the cover letter.

Responsibilities:
- Perform medical exams and comprehensive patient assessments and develop individualized care plans.
- Diagnose and treat common illnesses, injuries, and chronic medical conditions.
- Initiates treatment by appropriately prescribing medication, ordering diagnostic tests, exams, and procedures
- Reviews lab results ordered by NP and Physicians and propose plan of action according to results, communicate results.
- Perform procedures within the scope of practice, such as suture removal, wound care, and point of care testing.
- Coordinate care and perform routine health maintenance for established and new patients including telephone communications.
- Provide preventive health services by counseling and educating patients and families
- Monitor the effectiveness of therapeutic interventions and treatment plan.
- Make appropriate referrals.
- Document all services, procedures, and diagnoses within the electronic health record system in accordance with established standards and timeframe, to ensure accurate billing to patient accounts.
- Participate in regular team meetings and independent review and continuing education to stay current on healthcare trends and best practices.
- Provide excellent patient communication and foster a positive patient experience.
- Assist in development, revision and implementation of practice guidelines and directives, risk management and quality assurance policies and procedures. Carry out improvement initiatives as directed by partners/administrator(s).
- Provide medical care to all patients as delegated by physicians while utilizing independent judgment, and within delineated Practice protocols, guidelines, and directives.
- Other duties as assigned.
Qualifications:
- Master's or Doctorate degree in Nursing from an accredited Nurse Practitioner program.
- Current state NP license and certification or in the process of getting one.
- Previous experience in primary care or a related field preferred.
- Strong clinical and diagnostic skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a team.
- Proficient in using electronic health record (EHR) systems preferred.
- Dedication to providing compassionate and patient-centered care.
