Reports To:  Supervisor of Rental Assistance Program

Summary Description: The Housing Case Manager plays a vital role in providing comprehensive support to individuals in securing and maintaining stable housing. This position involves collaborating with clients, community resources, and internal teams to address housing needs and promote overall well-being.

Key Responsibilities:

  • Conduct housing assessments and develop individualized housing plans for clients.
  • Assist clients in accessing affordable housing options and navigating the application process.
  • Collaborate with property management, landlords, and housing agencies to secure housing opportunities.
  • Provide ongoing support to clients in maintaining stable housing and addressing any housing-related challenges.
  • Facilitate housing-related workshops and educational sessions for clients.
  • Coordinate with social service agencies, healthcare providers, and community resources to ensure holistic support for clients.
  • Advocate on behalf of clients to address housing-related barriers and challenges.
  • Maintain accurate and up-to-date documentation of client interactions, housing plans, and progress.
  • Participate in interdisciplinary meetings and case conferences to discuss client needs and progress.
  • Stay informed about housing regulations, policies, and resources to provide up-to-date information to clients.

Requirements:

  • Bachelor’s degree in Social Work, Psychology, or a related field preferred
  • Minimum of 2 years of experience in housing case management or related roles.
  • Knowledge of housing regulations, resources, and community services.
  • Familiarity with electronic case management systems.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a collaborative team.
  • Commitment to cultural competence and diversity.
  • Possession and maintenance of a valid Driver's License with a satisfactory driving record according to our insurance carrier's standards.
  • Must maintain current NYS Defensive Driving certification within 30 days of hire.
  • Must maintain pre-employment clearances by applicable regulatory clearances as required.
  • Legal eligibility to work in the United States.

Greater Mental Health Benefits

  • Salary Range: $44K- $46K, based on departmental budget, qualifications, experience, education, and licenses. This range represents the agency’s good faith and reasonable estimate of the range of possible compensation at the time of posting.
  • Flexible Work Arrangements: Subject to business needs, including hybrid options.
  • Paid Time Off: 25 days of PTO, 12 sick days, and 11 company-paid holidays per year (prorated for part-time staff).
