General Summary

At UMIT Home Health we go the extra mile in providing our patients with top-quality, convenient, consistent, and reliable in-home health care that’s personalized to specific needs. We are located near our patients to deliver timely care with an authentic approach. We service the following areas: Caroline County, Charles City County, Essex County, Hanover County, Henrico County, James City County King and Queen County, King William County, Middlesex County, New Kent County, Richmond County, and Richmond City.

The occupational therapist is responsible for providing skilled occupational therapy services, on an intermittent basis, to patients, in their homes, in accordance with physician orders. The therapist will provide initial evaluation, develop care plans, supervise COTAs, and communicate progress with physicians. The therapist will evaluate the patient’s home situation to determine instruction, family assistance, and other agency and community services that are needed to care for the patient. Territory for this position is primarily areas south of the James River but could cover other areas as needed. Pay will be competitive base salary.

PRIMARY DUTIES & RESPONSIBILITIES:
  • Evaluate patient's level of function within the patient’s home setting.
  • Conduct comprehensive assessments of the patient’s needs initially and periodically.
  • Develop & implement the interdisciplinary patient care plan including function-oriented goals that maximize patient safety, independence, & mobility within the home.
  • Ensure the provider’s orders are appropriate & update provider as necessary.
  • Communicate with the provider & clinical team regarding patient's status.
  • Builds trusting, collaborative relationships with multi-disciplinary team to support patient care needs.
  • Actively participates in multi-disciplinary team meetings, in-services, and case conferences to include identifying new OT referrals.
  • Utilizes evidence-based guidelines & objectively measurable tests to document progress.
  • Complete clinical progress notes within 24 hours.
  • Instruct/train patients and caregivers in home exercise programs, therapeutic ADLs/IADLs & creative activities that improve independence and physical/mental functioning.
  • Educate/train patients in use of ADL devices and durable medical equipment.
  • Identify possible ineffective drug therapy, adverse reactions, significant side effects, drug allergies, and contraindicated medications. Reports such immediately to ordering provider.
  • Confirm patients’ scheduled visits evening before visit.
  • Actively maintain continuing education requirements.
  • Assume other duties as deemed appropriate, compliant, & necessary by the clinical manager.
  • Maintain productivity expectations.
  • Performs other related duties as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIREMENTS:
  • Minimum bachelor’s degree in occupational therapy from an accredited program.
  • License to practice as Occupational Therapist within the state.
  • Valid CPR certification. Online certification is not acceptable.
  • At least 2 years of home health experience.
  • Strong computer and documentation skills.
  • Excellent understanding of performance improvement, quality assurance, and utilization management.
  • Demonstrate autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
EDUCATION and/or EXPERIENCE:
  • Minimum bachelor’s degree in occupational therapy from an accredited program.
  • License to practice as Occupational Therapist within the state.
  • Valid CPR certification. Online certification is not acceptable.
  • At least 2 years of home health experience.

OTHER REQUIREMENTS:

  • Valid Virginia driver’s license, acceptable driving record, and reliable transportation with insurance.
  • Applicants will be required to authorize a background check and submit to drug testing. Failure to do so will disqualify applicants from consideration for the position.

WORK ENVIRONMENT:

The work environment characteristics described here are representative of those an employee encounters while performing the primary functions of this position. Candidate will primarily work within diverse patient homes that may or may not be ADA compliant.

General office work is required. May be required to travel occasionally to attend meetings and conferences.

Physical duties include the ability to lift up to fifteen (15) pounds. Specific vision abilities required by this job include close vision, depth perception and ability to adjust focus. While performing the duties of this job, the employee is frequently required to stand, walk, and sit for long periods of time.
