Goshen, IN

Salary Range - $60,000 - $75,000

Full benefit package

Responsible to: Director of Nursing

 

Position Requirements: A. Education & Experience

1. Licensed as Licensed Practical Nurse by State of Indiana. Must maintain license as required by the facility or the state.

2. At least six months as a company nurse, or three-plus years of work as a nurse in a geriatric setting.

Job Description

Scope of Responsibility: Ensure that the residents receive the best possible clinical care through guidance of the clinical team. Monitor all activity assuring that state regulations are adhered to through resident clinical care and documentation. Believe in the values of customer-centered service, stewardship, ethical conduct and reflection, open communication, and fair treatment of all residents, family, and team members. Also responsible to the residents and families served.

Essential Functions: Rating (for evaluation purposes only)

Carry out duties and functions to support the campus as assigned.

Live the mission and values to improve the quality of services for residents and team members on the campus.

Respect residents’ rights and adhere to rules of resident care and services as described in the Communities Team Member Handbook.

Assure completion of all resident assessments and attend and participate in admission meetings as they pertain to your assigned community and/or as needed.

Ensure that incident reports are completed according to ISDH and company guidelines in a timely manner.

Participate in and ensure monthly care plan meetings adequately address issues related to resident care and care concerns also weekly Med A care plans.

Track and ensure completion of Med A certifications, Med A care plan meetings, Med A therapy meetings, and be responsible for other insurance-related concerns.

Attend and participate in monthly behavior meetings and follow up on any recommendations as they pertain to your assigned community and family involvement. Work with agencies to ensure recommendations are implemented.

Track and report weekly weights and report weights to the resident=s physician. Assure weights are completed timely by the RCS/CNA team.

Track and report significant blood sugars and report results to the resident=s physician.

Assist clinical nurses to problem solve, develop solutions, work efficiency and in developing time management skills. Coordinate resident care issues and assure appropriate paper compliance.

Train clinical team for adherence to state regulations and guidelines, policies, and procedures related to clinical care of the resident. Ensure clinical care on the community meets these established guidelines.

Coordinate resident care concerns with the community nurse and RCS/CNA team.

Identify any concerns relative to RCS/CNA caregiving practices and/or community nurse clinical care practices and convey serious and/or continued issues to the Director of Nursing.

Provide team member feedback concerning appropriate caregiving issues.

Assist with Quality Improvement audits and initiatives.

Make routine rounds and address team members, safety, cleanliness, and clinical concerns on your unit or shift.

Review twenty-four report and follow up.

Assist in Dining Room during meals.

Provide redirection and counsel team members as appropriate.

Complete team member feedback sessions, both annual and 90-day, as assigned.

Participate in the on-call schedule for weekends and holidays as assigned.

Coordinate and oversee the 24/7 clinical care functions of the unit. Must be available as needed to address unit needs and team member concerns.

Is responsible for building and maintaining a cohesive team environment.

Continually looking for process improvement opportunities.

 

Skills and Requirements 1. Strong written and verbal communication, problem-solving, and clinical skills.

2. Knowledge of Medicare regulations, ISDH regulations, and MDS Care Plans.

3. Ability to be creative in solving problems with both team members and residents.

Physical/Mental Requirements & Working Conditions

1. Frequent fingering, walking, sitting, talking, hearing, and repetitive motions required.

3. Requires the ability to lift up to 80 pounds occasionally and up to 60 pounds frequently.

4. Requires the ability to see things clearly at 20 inches or less, as is common in visual inspection or computer work.

5. This position has occupational exposure to blood-borne pathogens and other potentially infectious materials. Adherence to universal precautions is required.

6. Requires the ability to lift residents to a chair with the assistance of another person and a gait belt.
