What Your Role Will Be:
- Support individuals to reach their personal goals, from daily tasks to personal achievements
- Provide transportation to fun activities, medical appointments, and more, ensuring individuals stay connected with their community
- Support behavioral modifications and communication improvements, helping individuals thrive in every aspect of their lives
- Work with the individuals and other members of the team to develop service plans that aid the person in achieving their life goals.
- Support and promote the individual’s health and safety
- Maintain accurate and thorough documentation of service activities, including daily progress notes, interactions, and any additional information as required by agency guidelines.
- Assist with personal care, social interactions, meal prep, grocery shopping, and light housekeeping
Minimum Qualifications:
- High School Diploma or GED required
- Valid NYS Driver License with a minimum 1-year driving experience required
- Experience as a Caregiver, Direct Care Worker, Nursing Assistant, Home Care Aide or Companion a plus
- Experience with people with intellectual and developmental disabilities preferred
- Must be able to operate Motor Vehicle
- Evening and weekend shifts preferred
- Must be able to lift up-to 50 lbs.
Work Location: Residential houses located in Nassau County and Suffolk
Work Hours/Schedule: Full Time, Part Time & Per Diem. Day, Evening & Overnight shifts available.
Pay: $18/hour
