Individualized Home Support

Job Summary: We are seeking dedicated and compassionate individuals to join our team as Individualized Home Support (IHS) workers. In this role, you will help clients develop life skills, problem-solving, and coping skills to promote their independence both at home and in the community. IHS services are provided with or without training, and may include family training.
Responsibilities:
- Assist clients with in-home and community-based tasks, such as safety skills, paperwork, accessing resources, budgeting, and organizing.
- Help with home management activities, such as health and wellness, household management, and adaptive skills.
- Monitor client well-being and ensure safety in the home and community.
- Provide transportation and accompany clients to appointments within a 30-mile radius.
- Assist clients with personal care tasks as needed, such as grooming, dressing, and hygiene.
- Help clients develop coping mechanisms and problem-solving skills for daily challenges.
- Support clients in building social skills and participating in community activities.
- Document and report changes in client behavior, progress, and health to the Qualified Professional.
- Coordinate with family members, healthcare providers, and other support systems to ensure comprehensive care.
- Assist clients in creating and following a daily routine that promotes independence.
- Accompany clients to medical appointments, social activities, and errands.
- Offer emotional support and encouragement to clients to help them achieve personal goals.
- Teach and reinforce independent living skills, including meal preparation and medication management.
Qualifications:
- Must be at least 16 years old.
- Ability to work independently and follow care plans.
- Strong communication, dependability, and problem-solving skills.
- Must pass a criminal background check.
- U.S. Citizenship or valid work permit required.
- Must complete initial and annual training.
- Must have a valid driver’s license and current auto insurance to provide transportation services within a 30-mile radius.
- Experience working with individuals who have mental or physical disabilities is preferred. Candidates should demonstrate the ability to provide compassionate, patient, and adaptive support tailored to individual needs.
- Demonstrated ability to ensure client safety through effective risk assessment and proactive prevention strategies.
- Experience in managing crises calmly and effectively, with a proven ability to make quick, informed decisions in challenging situations.
- Strong attention to detail in monitoring and documenting client needs, behavior, and progress to ensure accurate and timely care.
- Excellent tact and judgment when interacting with clients, family members, and professionals, maintaining professionalism and sensitivity in all situations.
Physical Requirements:
- Ability to perform physical tasks such as standing, walking, kneeling, lifting up to 50 lbs.
- Comfortable working in a variety of home and community settings, including exposure to different weather conditions and behaviors.
- Ability to assist with client transfers, such as moving from a wheelchair to a bed or chair.
- Frequent bending, twisting, and reaching to assist with daily tasks and care activities.
- Ability to stand for extended periods of time while assisting clients in the home and community.
- Comfortable with handling and lifting assistive devices such as walkers, wheelchairs, or other mobility aids.
- Ability to push or pull clients in wheelchairs, including up and down ramps or stairs.
- Physically capable of assisting clients with outdoor activities, such as walking or accompanying them to parks and other community areas.
