Strive Health Services, LLC | Pediatric Physical Therapist

- Assess home health pediatric patient for rehabilitative and functional status.
- Regularly re-evaluate the patient's/client's physical therapy need and evaluate the outcomes of care on a regular basis.
- Physical Therapist develops, initiates, and revises the plan of care as necessary to ensure quality and continuity of care.
- Identifies deficits including but not limited to muscle strength / coordination, mobility, gait, ROM and potential for intervention. Evaluates the home environment.
- Physical Therapist evaluates the need for and recommends DME devices and aides.
- Uses infection control measures that protect both the staff and the patient (OSHA).
- Coordinates services. Informs the physician and other personnel of changes in the patient's/client's condition and needs.
- Physical Therapist demonstrates commitment and professional growth by participating in in-service programs and maintaining or improving competency.
- Supervises, teaches, and provides clinical direction to other physical therapy personnel as required or requested.
- Promote Agency philosophy and administrative policies to ensure quality of care.
- Physical Therapist provides effective communication to patients/clients, their family members, team members, and other health care professionals. *Targeting Licensed Physical Therapists with availability in the following locations:*
- Austin and surrounding communities, North (Pflugerville, N. Austin, Round Rock, Cedar Park, Leander)*QUALIFICATIONS*
- Graduate of an accredited Diploma, with Bachelor's, Master's, or Doctorate degree
- One year of experience as a Physical Therapist in a pediatric clinical care setting preferred
- Pediatric Home health experience preferred
- Reliable transportation
