Job Title: Medical Social Worker (MSW)
Duration of Assignment: 13 weeks
Location: Mission Viejo, CA
Shift: Day 5x8-Hour (08:00 - 16:30)
Salary Range: $2,400-$2,600 Weekly
Start Date: January 6, 2025

Job Duties:

  • Conduct patient assessments, care planning, and discharge coordination.
  • Address mental health crises, homelessness, and substance abuse.
  • Facilitate family meetings and manage protective services processes.
  • Arrange DME, SNF placements, and ambulance transport independently.

Requirements:

  • Master's in social Worker
  • Active California license.
  • Minimum 1 year of acute care experience.
  • Ability to work holidays and complete pre-start training modules.
