Essential Functions & Responsibilities
- FT telemed providers are required to maintain a minimum of 5 licenses in states where Theoria actively provides care. At any time Theoria may request providers to apply for and obtain a new license at the expense of Theoria. Multi-state licensure is a requirement of the telemed position.
- The provider is responsible for managing the clinical care of the population at the assigned SNFs.
- The provider is responsible for monitoring message boards, answering calls, and communicating with assigned SNFs during shifts.
- Provider will provide high-quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
- The provider is responsible for timely documentation of all patient care/encounters
- The provider will be available for phone/secure messaging consultations with facility nursing staff.
- Provider documentation is completed in Theorias proprietary electronic health record (EHR) system, ChartEasy.
- PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems.
- The provider is expected to be ready and on time for their remote scheduled shifts.
- Timely attendance is imperative.
- Reliable Wi-Fi is required.

Shift Structure
- Full time day shift is Monday through Friday 1PM-7AM and 2 12-hour weekend shifts per month, 7AM-7PM, during which the provider must be available to nursing staff and address acute/emergent issues that arise.
- Full time night shift is 15 shifts per month 7PM-7AM, during which the provider must be available to nursing staff and address acute/emergent issues that arise.
- Shift times are in EST
- Charting/documentation can be completed after the shift but within 48 hours
- This is a remote position.
