Pharmacist - Full Time - San Fernando

RPh on the Go is seeking a highly skilled and motivated Inpatient Pharmacist with hospital experience to join our healthcare team in California. The successful candidate will be responsible for ensuring the safe and effective use of medications in an acute care setting, collaborating with healthcare providers, and providing direct patient care to optimize therapeutic outcomes.
Shift: 8:00pm 6:30am
Key Responsibilities:
Review and verify medication orders for accuracy, appropriateness, and potential interactions.
Collaborate with physicians, nurses, and other healthcare professionals to develop and implement patient care plans.
Monitor patient response to therapy, adjust treatment as needed, and provide medication counseling.
Manage medication distribution, compounding, and sterile preparation in compliance with state and federal regulations.
Ensure compliance with hospital policies, procedures, and accreditation standards.
Qualifications:
Active California Pharmacist license in good standing.
Doctor of Pharmacy (PharmD) degree from an accredited institution.
Minimum of 2 years of inpatient hospital pharmacy experience.
Strong knowledge of pharmacokinetics, medication safety, and hospital pharmacy protocols.
Excellent communication, critical thinking, and problem-solving skills.
Experience with electronic health records (EHR) and automated dispensing systems is preferred.
