Compelling Opportunity for Home Health Field Clinician

As a team member of our Senior Community Care (SCC) product, you will be part of an interdisciplinary team providing care to patients at home or in facilities.

This role offers a unique chance to make a meaningful difference in people's lives by improving access to care and creating a seamless health journey for patients across various care settings. You will have the opportunity to work collaboratively with healthcare professionals, including Advanced Practice Clinicians (APCs) and Patient Care Coordinators (PCCs), to deliver high-quality care to our highest-risk health plan members.

The Senior Community Care (SCC) Program is a comprehensive, longitudinal care delivery program that coordinates clinical care for members in facility settings. As an Optum Care Team RN within SCC, you will provide care to our most vulnerable population, working closely with your team to ensure accurate and complete documentation, effective communication, and seamless coordination of care.

Your Key Responsibilities:
  • Ensure accurate and complete documentation of patient information.
  • Communicate and collaborate effectively with the interdisciplinary care team.
  • Collaborate with PCCs to support APC care and scheduling of members.
  • Assist providers with various care coordination activities.
  • Support benefit determination associated with Medicare Part A, B, and D benefits.
  • Identify and respond to patient changes in condition, collaborating with primary APCs and facilities.
  • Coordinate therapy sessions and participate in facility-based meetings.
  • Schedule and participate in family conferences, team meetings, and case presentations.
  • Coordinate training, track quality measures, and assist in creating in-services for selected audiences.

About You:
You are a highly skilled Registered Nurse with experience in acute care settings. Your ability to work independently and as part of a team, combined with your strong communication skills, makes you an excellent fit for this role. We require:
  • An Associate degree or higher in Nursing (RN).
  • An active, unrestricted license in the state where you reside.
  • Certification in Basic Life Support.
  • The ability to complete physical requirements, including lifting a 30-pound bag and performing physical assessments.
  • Access to reliable transportation.
  • The ability to travel between facilities.

Salary and Benefits:
In New Jersey, this per diem role comes with a competitive hourly rate of $$28.03 to $54.95 per hour.
