Key Responsibilities:

  • Conduct in-home health assessments with senior patients, documenting vital signs and identifying cognitive, functional, medication, and social challenges impacting patient health.
  • Communicate health updates with providers and virtual care teams, facilitating care plan execution and design to keep patients safe and healthy.
  • Prepare patients for virtual examinations, perform routine screenings, and assist providers with exams and treatment procedures.
  • Administer medications, change dressings, apply bandages, remove sutures, and perform first aid procedures as needed.
  • Interact with EMS and emergency department personnel to coordinate emergency care for patients requiring hospitalization or advanced life support services.
  • Document clinical findings, medications, allergies, and medical conditions, ensuring accurate and timely reporting to providers.
  • Maintain home visit supplies, equipment, and stocks, adhering to OSHA safety standards and accurate documentation practices.

Requirements:

  • Current unrestricted Paramedic FL state license
  • Minimum 4 years work experience as a Paramedic or 4 years EMT experience in public health or health-related fields
  • Community paramedic certification (CP-C) and ALS certification preferred
  • Experience in telemedicine or virtual care delivery and working with geriatric patients a plus
