Masonicare Hospice Music Therapist Position Overview

The Masonicare Hospice Music Therapist provides direct services to hospice and palliative care patients and their families in homes and inpatient and outpatient units.

Key Responsibilities:
  • Assess the needs of patients and provide interventions that meet individual needs, collecting data in collaboration with the patient and/or family as demonstrated by documentation on the patient record.
  • Perform initial assessments of needs during first visits and document those needs within appropriate time frames, assessing emotional well-being, physical health, social function, communication abilities, and cognitive skills through musical responses.
  • Communicate findings and problems to appropriate team members, planning and implementing care for patients based on needs assessed to ensure individualized care.
  • Identify and document problems, setting developmentally appropriate goals, time frames, and interventions, providing and documenting direct patient care according to the Care Plan within set target dates and according to all departmental policies.
  • Evaluate patient progress, taking prompt action when any change in the patient's condition is noted or reported, and terminating care when goals of management are attained for patients of all ages.

Requirements:
  • Bachelor's degree in music therapy or related field; Master's degree preferred
  • Graduate of an approved music therapy program; Board Certified as a Music Therapist
  • Minimum two years experience working in end-of-life issues preferred
  • Maintains current Drivers' License and auto insurance
