Salary: $65,000 - $85,000 per year, depending on experience

About the Role

We are seeking an experienced RN Case Manager to join our team in Augusta, GA. As a key member of our Integrated Kidney Care team, you will work closely with patients, families, and healthcare providers to deliver high-quality care and improve patient outcomes.

The ideal candidate will have at least 2 years of experience in case management or chronic care management, with a strong background in medical record review, consultation, and coordination of care. You will be responsible for identifying the medical, social, emotional, and financial needs of patients with complex conditions and implementing effective interventions to address these needs.

This is a hybrid remote position, requiring occasional travel within an assigned territory for in-person visits. You must be flexible and able to accommodate patient and nephrology partner's availability.

Requirements
  • Current RN license
  • BSN preferred
  • Minimum of 2 years of RN experience with healthcare performance coaching
  • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
  • Intermediate computer skills and proficiency in MS Word, Excel & Outlook
