We are seeking a highly skilled and compassionate Advanced Practice Registered Nurse to join our team. This role presents an exceptional opportunity to deliver high-quality patient care while working in various settings, including home-based care, assisted living facilities, and independent living communities.

Salary Range: $100,000 - $120,000 per annum

About the Role
  • Deliver evidence-based medical care to patients with complex needs, ensuring their comfort and well-being in their preferred environment.
  • Conduct thorough examinations, diagnose conditions, and develop personalized treatment plans.
  • Collaborate with caregivers and other healthcare professionals to ensure seamless patient care and effective communication.
  • Maintain accurate patient records, documenting all interactions and treatments.
Requirements
  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
