Job Summary:
The Licensed Practical Nurse (LPN) will provide nursing supports to individuals with intellectual disabilities, working under the supervision of a Registered Nurse. The ideal candidate will have a strong background in delivering healthcare services and a passion for working with individuals with special needs.
Key Responsibilities:
- Assist with direct care of individuals as needed, including reviewing staff documentation and taking appropriate action when indicated.
- Complete nursing notes as needed, including significant medical changes, interventions, and communications with providers.
- Monitor individuals' medical needs and health status on a continuous basis, providing treatments and care, and documenting medical histories.
- Ensure that individuals' prescriptions are filled/refilled, record physician's medication orders, and monitor medication administration sheets to ensure proper administration.
- Review clinical data and upload medical information in My Health Update and Evero.
Requirements:
- Associate degree in Nursing and two years of professional experience in delivering healthcare services to individuals with intellectual disabilities.
- Licensed as a Licensed Practical Nurse required.
- Excellent presentation and writing skills, with the ability to translate agency philosophy into practice and implementable techniques and interventions.
- Knowledgeable in Electronic Health Records and valid driver's license for travel to multiple locations.
