LHC Group is seeking a skilled Occupational Therapist to join our team in providing high-quality care to patients in their homes. As an Occupational Therapist, you will be responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. You will provide services within the scope of practice as defined by state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the healthcare team. Your responsibilities will include evaluating patient functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. You will consult with the physician in the development of the therapy plan of care and observe, record, and report to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition. To be successful in this role, you will need to have a current Occupational Therapy licensure in the state, current CPR certification, and a current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
