We are seeking a highly skilled Caregiver Clinician to join our team at Mass General Brigham Healthcare at Home. As a seasoned professional with a background in acute care, you will provide quality clinical services to patients in various settings, delivering care that is tailored to their unique needs and circumstances.

About the Role

Your primary responsibility will be to provide skilled nursing care to patients in their homes, working collaboratively with other healthcare professionals to ensure that they receive the highest level of care possible.

In this role, you will be responsible for:

  • Providing comprehensive assessments of patients' physical and emotional needs, developing personalized care plans to meet those needs.
  • Delivering high-quality, evidence-based care to patients in their homes, working collaboratively with other healthcare professionals as needed.
  • Maintaining accurate and detailed records of patient interactions, including progress notes and medication administration records.
  • Communicating effectively with patients, families, and healthcare teams to ensure seamless care coordination and transitions.
